Thank you for your interest in Brooks Equipment Company. We are excited that you have chosen to work with us in your endeavors to help grow your business.

Brooks sells only to distributors and service companies, never the end-user, and to ensure that we only open accounts with those who service or sell within the fire and life safety industries - as well as offer you the best customer service and credit terms - we ask that you complete the attached forms so that we may better understand your company’s profile and needs.

A few key things to remember:

• Please fill out the New Customer Application and New Customer Profile forms completely.
  
• If you have a resale tax number, please fill out our Resale Tax Certificate form and sign it. If you choose to pay sales tax on your purchases, please attach a copy of your business license.
  
• If you are applying for Net 30 Day Terms, please fill out the Application for Credit and Major Trade References forms. Be sure to sign both forms where designated.
